You can help shape the future of the new Community Health and Wellness Center! In November 2023, Bloomington voters passed a ballot initiative authorizing a half-percent local option sales tax to pay for three community amenities, including construction of a new Community Health and Wellness Center. Public input is crucial to ensuring the new facility meets current and future community needs. The City of Bloomington is conducting a survey to help determine the recreation, health, and wellness priorities for Bloomington residents to meet our strategic priorities of creating a connected, welcoming and healthy community. These items contribute significantly to our quality of life in Bloomington, and your opinions are very important to us.
Your household is one of a limited number selected at random to receive this survey, therefore, your participation is key. This survey will take approximately 15-20 minutes to complete. Each question is important. The time you invest in completing this survey will aid the City in taking a resident-driven approach to facility design and recreation, health and wellness program planning.
ETC Institute, an independent consulting company, is our partner in administering this survey. They will compile the data received and present the results to the City of Bloomington. Your responses will remain confidential and will be reported in group form only. Upon completion of the survey, you will have an opportunity to opt-in for a chance to win one (1) $500 prepaid Visa gift card as a thank you for your assistance.
